RESUMO

Objective:Hybrid attention U-net (HA-U-net) neural network was designed based on U-net for automatic delineation of craniospinal clinical target volume (CTV) and the segmentation results were compared with those of U-net automatic segmentation model.Methods:The data of 110 craniospinal patients were reviewed, Among them, 80 cases were selected for the training set, 10 cases for the validation set and 20 cases for the test set. HA-U-net took U-net as the basic network architecture, double attention module was added at the input of U-net network, and attention gate module was combined in skip-connection to establish the craniospinal automatic delineation network model. The evaluation parameters included Dice similarity coefficient (DSC), Hausdorff distance (HD) and precision.Results:The DSC, HD and precision of HA-U-net network were 0.901±0.041, 2.77±0.29 mm and 0.903±0.038, respectively, which were better than those of U-net (all P<0.05). Conclusion:The results show that HA-U-net convolutional neural network can effectively improve the accuracy of automatic segmentation of craniospinal CTV, and help doctors to improve the work efficiency and the consistent delineation of CTV.

RESUMO

ObjectiveTo study the intervention of Huanglian Jiedutang on atherosclerosis (AS) in apolipoprotein E knockout (ApoE-/-) mice induced by the high-fat diet. MethodThe ApoE-/- mouse model of AS was induced by the high-fat diet, and Huanglian Jiedutang was used to intervene in the AS in the ApoE-/- mice. The pathological changes of aorta were observed by hematoxylin-eosin (HE) staining. The levels of serum total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) were detected by an automatic biochemical analyzer. The protein expression levels of sirtuin-1 (SIRT1) and nuclear factor-kappa B (NF-κB) were determined by Western blot assay, and the mRNA expression levels of adenosine 5'-monophosphate-activated protein kinase (AMPK), peroxisome proliferators-activated receptors α (PPARα), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin-1β (IL-1β), and NOD-like receptor pyrin domain-containing 3 (NLRP3) were determined by real-time quantitative polymerase chain reaction (Real-time PCR). ResultAs compared with the normal group, there was a large amount of lipid accumulation in the blood vessels of the model group. In the model group, the levels of serum TG, TC, and LDL-C were increased (P<0.01), and the level of HDL-C was decreased (P<0.01). The protein expression level of SIRT1 in the aorta was decreased, while that of NF-κB was increased in the model group (P<0.01). The mRNA expression levels of IL-6, TNF-α, and IL-1β were higher (P<0.01), while those of AMPK in the liver were lower in the model group (P<0.01). Compared with the model group, the Huanglian Jiedutang group reduced the lipid accumulation and inflammatory reaction in the aorta of mice with AS, reduced the levels of TC, TG, and LDL-C (P<0.01), and increased the level of HDL-C (P<0.01). Huanglian Jiedutang significantly increased the protein expression level of SIRT1 in the aorta of ApoE-/- mice (P<0.01) and decreased the protein expression levels of NF-κB in the aorta (P<0.05, P<0.01). Huanglian Jiedutang down-regulated the mRNA expression levels of TNF-α, IL-6, IL-1β, and NLRP3 in the aorta (P<0.05, P<0.01), and up-regulated the mRNA expression levels of AMPK and PPARα in the liver of ApoE-/- mice (P<0.05, P<0.01). ConclusionHuanglian Jiedutang has a certain intervention effect on the formation of atherosclerotic aortic plaque in ApoE-/- mice. Its mechanism may be related to the decrease of serum TC, TG, and LDL-C levels, the increase of HDL-C levels, thus playing a role in lowering blood lipid, the increase of SIRT1 protein, the decrease of NF-κB protein, the decrease of inflammatory factors such as TNF-α and IL-6, which protects blood vessels from inflammatory injury, and the improvement of AMPK and PPARα levels to participate in autophagy and apoptosis.

Objective:To preliminarily explore the effects of tumor treating fields (TTF) arrays on the dose distribution in the treatment of Glioblastoma (GBM) using combined radiotherapy and concurrent TTF.Methods:EDR2 and MatriXX plate ionization chamber were employed to measure the absorbed doses of tissues at different depths (< 1 mm, 3 mm, 5 mm, 1 cm, 1.5 cm, 3 cm, 5 cm, 10 cm, and 15 cm) in the case that TTF arrays and latex-free foam were attached and not attached on the surface. Then the absorbed doses were calculated, compared, and analyzed. For the volumetric arc therapy (VMAT) of 10 GBM patients, deep dose verification was performed using the Sun Nuclear ArcCheck 3D dose verification system and the D99%, Dmean, and D1% of tumors and OARs were assessed. Results:The surface dose increased by 173% in the case that TTF arrays and latex-free foam were attached to the surface compared with the case of the surface with nothing attached. The surface dose increased by 61.7% due to the attachment of low-density latex-free foam. The dose deviation gradually decreased with an increase in the depth and stabilized (about 4%) at a depth of greater than 1.5 cm. As indicated by the VMAT verification result, the D99%, Dmean, and D1% of PTV and CTV decreased by 1.1%-1.2% and the Dmean and D1% of OARs (i.e., brainstem, pituitary gland, optic chiasma, optic nerve, eyeball, and eye crystal) decreased by 0.7%-1.5% in the case that TTF array and latex-free foam were attached on the surface compared with the case the surface with nothing attached. Conclusions:The combined radiotherapy and concurrent TTF in the GBM treatment will lead to a slight reduction of the absorbed doses of targets and OARs but a significant increase in the absorbed doses of the scalp. Therefore, it is recommended that the scalp doses should be reduced as far as possible in the design of the radiation treatment plan to reduce the adverse reactions on the scalp of GBM patients.

BACKGROUND:Studies have found that both bone marrow mesenchymal stem cels and umbilical cord mesenchymal stem cels have therapeutic effects on liver failure, but little is reported on adipose-derived mesenchymal stem cel transplantation for treatment of liver failure. OBJECTIVE:To discuss the therapeutic effect of adipose-derived mesenchymal stem cel transplantation on rat liver failure. METHODS: Forty-five Sprague-Dawley rats were equaly randomized into transplantation, model and control group. Except the control group, animal models of liver failure were made by intraperitoneal injection of D-galactosaminein in the transplantation and model groups. One week after modeling, rats in the transplantation group were given 1 mL of adipose-derived mesenchymal stem celsvia the tail vein. RESULTS AND CONCLUSION:Adipose-derived mesenchymal stem cell transplantation successfully upregulated alanine aminotransferase and aspartate aminotransferase levels, and moreover, neatly arranged liver cells, reduced inflammatory cells and intact hepatic lobule were shown on B ultrasound after cell transplantation. All these findings indicate that adipose-derived mesenchymal stem cell transplantation has therapeutic effects on rat liver failure by improving rat’s liver function and pathological changes.
